Toddler’s death sparks urgent playset warning from feds

Aug. 13, 2026, 3:29 p.m. ET

Parents are being urged to stop using a popular toddler slide playset immediately after federal safety regulators linked the product to the death of a 17-month-old child.

The Consumer Product Safety Commission on Aug. 13 warned that the WPX-4 slide playset, manufactured by China-based Wenzhou Yidian Amusement Equipment Co., Ltd., contains openings that can trap a child’s head and neck, creating a risk of serious injury or death.

The agency said the playsets, which can be used indoors or outdoors and feature a slide, tunnel and basketball hoop, should be discarded immediately. The CPSC is aware of one fatality involving the product and said the manufacturer has not agreed to an acceptable recall.

Consumers are being advised against using a slide playset, model WPX-4, with the vehicle-shaped side component following the death of a 17-month-old child.

Do you own one of the playsets?

The plastic playsets were sold in multiple colors, including red, navy, coffee, gray, green and yellow. Some models feature a vehicle-shaped component on the side. Consumers can identify the products by looking for model number WPX-4 and the manufacturer’s name, Wenzhou Yidian Amusement Equipment Co., Ltd., on the product label.

The CPSC is urging consumers to stop using the playsets immediately and not to sell, donate or give them away. Anyone who experiences a product-related incident or safety issue can report it through the agency’s consumer safety database at SaferProducts.gov.
